A small dose of radiation therapy has beneficial effects on normal tissue sparing and tumour cell kill. A shorter course of radiation therapy is called hypofractionatedradiation therapy; where we give a higher dose of radiation therapy per fraction and the treatment days are fewer. In this modern day of busy schedules, shorter treatment days are beneficial for both the patients and their attendants.

However, this higher dose radiation therapy has its own issues and needs to be sorted prior to routine use in all patients. Fewer days of treatment has a quality of life advantage over long treatment. And patients have less depression during treatment after hypofractionated radiation therapy.

For a larger volume disease this shorter duration treatment may cause more toxicity and hence appropriate selection of cases for hypofractionated treatment or radiosurgery is important and is the most important factor that impacts on the outcome.

Short course radiation therapy (Hypofractionated radiation therapy) is the one of the most talked about subjects in recent years and also a fascinating research zone. Hypofractionated (short course) radiation therapy is an old concept, but only in recent years with tremendous improvement in radiation therapy delivery technologies is there a significant visible surge in its applicability in clinical practice. High dose- short course radiation therapy of one to five days is termed as ‘Radiosurgery’.

Gamma knife and CyberKnife are dedicated radiosurgery tools. Whereas Truebeam, Novalex Tx, Tomotherapy are linac based equipment that are mostly used for conventional treatment, but they can also be used treat patients with radiosurgery.

Among these equipment, the CyberKnife is an extension of Gamma knife, and at present, CyberKnife is the most modern and sophisticated dedicated radiosurgery equipment.
